An LPC-Associate is a Licensed Professional Counselor Associate who has completed the required graduate education and training to provide counseling services while working toward full licensure as a Licensed Professional Counselor (LPC).

At this time, I am a private-pay practice and do not accept insurance. If you’d like, I can provide a superbill that you may submit to your insurance company for possible out-of-network reimbursement.
Please note that most insurance companies require a mental health diagnosis for reimbursement. A diagnosis becomes part of your medical record and may be required for your insurance company to determine eligibility for out-of-network benefits. Because coverage varies by plan, I recommend contacting your insurance provider to learn more about your benefits and reimbursement policies.

Individual therapy sessions are $120 for a 50-minute session.
Couples and family therapy sessions are $135 for a 50-minute session.

Yes. I offer a limited number of reduced-fee options for clients who may need financial assistance. Availability is based on current openings and financial need.
I also accept referrals through Openpath and the Mental Health Liberation Foundation to help increase access to affordable mental health services.

If you need to cancel or reschedule your appointment, please provide at least 24 hours’ notice. Appointments canceled with less than 24 hours’ notice, or missed appointments without notice, will be charged the full session fee.

If you are experiencing a mental health emergency, feel that you may harm yourself or someone else, or are having thoughts of suicide, please seek immediate support. Armadillo & Sage Counseling, PLLC is not a crisis or emergency service and I may not be immediately available outside of scheduled appointments.
If you are in immediate danger or need urgent assistance:
Call or text 988 to reach the Suicide & Crisis Lifeline (available 24/7).
Call 911 or go to your nearest emergency room if you are experiencing an emergency or cannot remain safe.

Texas law requires that requests for mental health records be made in writing.
To request a copy of your records (or your child’s records, if applicable), please follow the steps below.
Submit a Written Request
You may submit your request using one of the following methods:
Via Client Portal
If you have access to the client portal, you may send me a secure message requesting your records. I will provide you with any required HIPAA-compliant Authorization for Release of Information form, if applicable.
Please include:
The specific records you are requesting.
The name, mailing address, and/or email address of the person or organization that should receive the records.
Via Email
If you do not have access to the client portal, you may email your written request and any required Authorization form to:

Records Requested for Litigation
If records are being requested for use in litigation, please include:
Cause number
Case title
Court where the case is pending
Business Records Affidavit
If you require a Business Records Affidavit, please indicate this in your request.
Any applicable fees associated with preparing requested documents or affidavits will be communicated prior to completion. Affidavits will not be provided until payment, if applicable, has been received.
Records Fees
If there are fees associated with copying, preparing, or providing records, I will notify you of any applicable charges before processing your request.
Records are not required to be released until applicable fees, if any, have been paid.
Couples, Family, or Third-Party Records
If you are requesting:
Couples counseling records,
Family counseling records, or
Records for someone other than yourself or your minor child,
state and federal law require either:
A valid court order, or
A written Authorization signed by the individual whose records are requested (or the individual’s parent or legal guardian, when applicable).
Relevant Law: 45 C.F.R. § 164.512(e); Texas Health & Safety Code §§ 611.004, 611.0045, and 611.008.
Information for Attorneys and Records Request Companies
A subpoena alone is generally not sufficient to require disclosure of confidential counseling records, billing records, or other Protected Health Information (PHI) under HIPAA.
Mental health records requested in connection with litigation, including requests for testimony, may only be released when permitted by law, including:
A valid court or administrative tribunal order; or
A valid written Authorization from the client (or parent/legal guardian); or
Another exception permitted under HIPAA.
Relevant Law: 45 C.F.R. § 164.512(e).
Important Notice
If a client (or the parent/legal guardian of a minor client) has not authorized the release of records:
A Statement of Assurance will not be accepted.
A valid Authorization or Court Order is required before records will be released.
